Advanced Clinical Practitioner

The Medical Centre, Driffield, is looking for an Advanced Clinical Practitioner to work 3 (27 hours) or 4 (37 hours) days per week. Driffield is a market town in the East Riding of Yorkshire surrounded by the picturesque wolds, with easy links to nearby York and Beverley and is approximately 10 miles from the beautiful Yorkshire coastline.

The main surgery is located in Driffield town centre with a branch surgery in the village of Wetwang, 6 miles west of Driffield. It is a semi‑rural practice with 12,500 patients, and we dispense to approximately 50% of our patients from Driffield and Wetwang.

Job Summary

The Advanced Clinical Practitioner will provide autonomous, accountable care within a holistic primary care setting. You will work in both the main practice and the care community, including care homes, and support a Total Triage model of appointment booking.
