Job Description
As part of our digital onboarding, we require a dedicated Contract Acceptance Form for our moving clients. This form must be legally robust, user-friendly, and provide a clear audit trail. Please implement the following requirements:
1. Header & Data Integration
Quote Sync: The form must dynamically pull and display the Quote Number, Move Date, Total Price, and Service Address. These fields should be read-only for the client.
Service Summary: Include a summary section showing the primary services agreed upon (e.g., Number of movers, Packing services, Storage).
2. Mandatory Selection Fields (Anti-Dispute)
Valuation/Insurance: Provide a mandatory Radio Button selection. The user must actively choose between Basic Liability and Full Value Protection. Do not pre-select an option.
Terms Acknowledgment: Implement a Scroll-to-Accept window for our Terms & Conditions. The Sign button should remain inactive until the user has scrolled to the bottom.
