Enhance resident experience as a Service Coordinator at Extendicare in Mississauga, Ontario. This full-time position is dedicated to advocacy and personalized care for residents in a supportive environment.
You will coordinate therapeutic services and contribute to the interdisciplinary assessment and care planning process. Your role is crucial in educating staff, families, and students on best practices in resident care and rights advocacy. Engagement with community partners will enhance the support provided to residents.
Key Responsibilities:
• Participate in resident admissions and transfers thoughtfully
• Provide individual and group counseling as needed
• Ensure proper coordination with Psychogeriatric Consultant
• Maintain accurate records and compliance documentation
• Develop in-service programs for staff and facilitate mentoring
