Occupational Therapist, North Devon - North Devon

Location The post is based within the North Devon Community Team, with flexibility to work across localities depending on service need. Our bases include South Molton, Holsworthy, Bideford, Barnstaple and Ilfracombe.

About the role As a Community Occupational Therapist, you will use core occupational therapy skills and clinical expertise to support adults with complex needs to maximise their independence and achieve outcomes that matter to them.

You will:

• Undertake strength-based, person-centred assessments in community settings, both face-to-face and via telephone.
• Lead on complex cases, including safeguarding, care planning and risk management.
• Recommend and provide equipment, environmental adaptations and reablement-focused interventions.
• Work collaboratively within multidisciplinary teams across health, social care, housing and the voluntary sector.
