Advanced Nurse Practitioner (or Trainee)

Our Acute Frailty team are seeking a highly motivated practitioner with a passion to improve the experience of our ageing and frail population. The post holder must have knowledge and prior experience of managing patients with high clinical frailty scores and undifferentiated complex care presentations. Excellent communication skills are essential to communicate with patients and carers regarding clinical conditions and experience in sensitive discussions around anticipatory care planning would be advantageous. The role may involve working across organisational boundaries and in partnership with other health and social care teams.

The successful candidate will join a team who utilise Comprehensive Geriatric assessment as a method of screening frailty and planning multi-disciplinary care.
