- Specialty: Geriatric Care - NURSING HOME - Advanced Practice Provider
- Schedule: 1.0 FTE Monday-Friday-between 6:30am and 5:00pm - Travels to assigned nursing home facilities, covering the Fox Valley from Appleton to Oshkosh (includes Kiel and Chilton).
- Call Schedule: 1:6, nights, weekends and holidays
- Practice Detail: Family Medicine practice in a nursing home environment. Provides onsite care to nursing home patients at various locations; works with a support team onsite to assist with patient care needs. Experience preferred. Visit 12+ patients per day in the skilled nursing facility. Responsible for schedule to assigned sites, or as needed.

Responsibilities
Ascension-Wisconsin is looking for a Board Eligible/Board Certified Advanced Practice Provider (Nurse Practitioner/Physician Assistant) to join our Nursing Home Team. This is a great opportunity for the right candidate to partner with Ascension-Wisconsin in the Fox Valley to provide medical services to patients.
Position Highlights:
- Call Schedule is 1:6, with no nights
Offers a busy schedule, travel within the Fox Valley, experienced practitioner preferred in Family Medicine

Responsibilities:
- In collaboration with a physician, provide for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions.
- Obtain patient history and perform assessment via observation, interview and examination. Order, perform and interpret diagnostic studies. Perform preventative health assessments, screening, immunizations and patient care.
- Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources.
- Counsel and educate patients and their families/caregivers concerning preventative health, treatment options and community resources.
- Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

Requirements
Licensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
- Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
- Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred. Verified by local Credentialing Office.
Education:
- Master's degree of Nursing required.
