Role Summary
Band 8a Advanced Practitioner, or band 7 Trainee Advanced Practitioner to work across our Acute Hospital at Home, Frailty and Hospital Outreach services in North Devon.
The role will require working in a variety of settings including the acute trust sites and patients own homes, in order to best support patients to remain safe and well at home, supporting alternatives to admission.

We are looking to recruit highly enthusiastic and motivated qualified Advanced Practitioners (AP) or trainee APs to be part of our Hospital Outreach, Acute Hospital at Home and Acute Frailty Services in North Devon. The successful candidates will work in accordance with the Trust frame work for Advanced Practice or be willing to undertake the training programme to achieve AP status.
