Aps6 Clinical Care Coordinator

DFP Recruitment is seeking a Clinical Care Coordinator to join our Federal Government clients' team on a long-term assignment (up to 24 months). This role is full-time hours (Mon - Fri) and will be based in Winnellie, NT .

The position of Clinical Care Coordinator involves working collaboratively with clients and internal and external stakeholders to improve the ability to identify and respond to the needs of vulnerable and at-risk veterans, current & former Defence Force members, and/or their families.

The Clinical Care Coordinator will adopt a strengths-based, recovery approach to support vulnerable and/or at-risk clients, enhancing their wellbeing and community participation through community engagement, collaborative recovery work, case coordination, and complex case management.
