Job Description Job Description We are seeking a Home Health Care Worker to become a part of our team! You will provide routine healthcare to patients at the patient's home. Responsibilities: Provide routine individualized care to persons with disabilities Monitor and report changes in patient health status Provide personal care, such as bathing and dressing patients Provide specialized care that focuses on non-medical services for members Record notes on me member progress on daily goals and the results Teach family members ways to care for patient Qualifications: Experience in hospice, home health, or other related field service Must have reliable transportation and clean MVR Positive and patient demeanor Ability to build rapport with patients Ability to handle physical workload Excellent written and verbal communication skills Quality Customer service to members Requirements : Must be have or be willing to obtain a fingerprint clearance card Stated Required Training must be completed within 30 days of hire Must be 18 years of age and older to be considered for employment Background is required for this role Company Description Northbound Health – accessible via www.nbhaz.com – is a Phoenix, Arizona–based home health care provider offering a range of in‐home therapeutic services.
