Job Description Care Coordinator Schedule: Monday–Friday, 8a–5p (minimal evenings/weekends) Focus: Completing assessments and initiating care plans for a high volume of new members. Contract Duration : 5 months (with possiblity of extension/conversion) Start Date: ASAP Summary: The Care Coordinator supports members by completing health and psychosocial assessments, developing initial care plans, and coordinating services with an inter-disciplinary care team (ICT). The role centers on telephonic/electronic outreach and ensuring members receive timely, culturally competent support. Key Responsibilities: Conduct health and psychosocial assessments. Develop and document individualized care plans with the ICT. Identify barriers to care and implement interventions. Educate members/caregivers on resources and benefits.
