Job Title: Care Coordinator
Job Profile Summary: Neural Care (NC) Navigators provide high-touch care navigation services for dementia patients and their caregivers using an AI-first platform. The NC Navigator onboards caregivers and patients to the NC platform, maintains regular check-ins via in-app chat or recorded calls (and occasional in-person meetings when appropriate), triages concerns, and coordinates solutions with clinical teams and community partners. The goal is to reduce caregiver burden by continuously understanding needs and ensuring the platform effectively meets them.
Key Responsibilities:
Onboarding & Relationship Building
Onboard caregivers and dementia patients onto the NeuralCare platform and ensure profiles are kept up to date.
Establish a regular check-in cadence; be available in person (as needed), virtually, or by phone/in-app chat (calls/chats recorded per policy).
Build trusting, longitudinal relationships with patients and care partners.
