Ensure plan adherence: monitor daily services, review progress toward goals, and retrain staff when updates are needed. Lead client advocacy: accompany clients to physician appointments or hospital visits; relay updates to the healthcare team and family members. Recruit & train caregivers: assist HR in hiring, onboarding, and educating field staff on best practices and agency protocols. Schedule like a pro: build and adjust caregiver schedules for a caseload of ~30-40 clients, balancing continuity of care and employee preferences. Track outcomes: document assessments, incidents, and goal progress in our EMR/CRM; generate reports that drive quality improvement. Problemsolve fast: resolve urgent issues with calm, confidence, and compassion-24/7 oncall rotation shared with team. What You'll Bring Certification / license: CMA, CNA, LPN, RN, or Social Worker credentials welcome (preferred but not required) 1-2 years' experience as a Care Coordinator, Client Care Manager, Case Manager, or similar role-homecare or healthcare setting highly valued Strong assessment, careplanning, and documentation skills; familiarity with HIPAA and state regulations Proficiency with Microsoft Office and EMR/CRM scheduling software Excellent verbal and written communication; you build trust quickly with clients and clinicians alike Stellar organization: juggle multiple priorities, meet deadlines, and adapt in a fastpaced environment Reliable transportation for client visits; valid driver's license Ready to Champion Client Care?
