Care Integration Manager, Hospice

BAYADA Home Health Care is currently seeking a Care Integration Manager to support our hospice team in Horsham, PA. Care Integration Managers will manage, and support the transition of complex, chronically ill clients between BAYADA Home Health and Hospice practices, using data to proactively advocate for and coordinate the utilization of the most appropriate benefit at the right time. This is a hybrid position, with weekly travel to Horsham, PA and travel through Bucks, Montgomery and Philadelphia counties, as-needed. Qualifications of the Care Integration Manager:

Four (4) year college degree.

Five (5) years’ experience in nursing, social work, care coordination, and care integration in a relative specialty (home health, hospice, etc.)

Excellent verbal and written communication skills.

Ability to work seamlessly across settings and practice lines in a highly matrixed local environment.
