📍 Location: Indiranagar, Bangalore
🕘 Work Timings: 9:00 AM - 6:00 PM or 10:00 AM - 7:00 PM
💰 Compensation: 5.5 LPA (Fixed) + ESOPs
🕘 Work Days: Monday - Friday (In-office) and Saturday (Work from Home)

Role Summary: This role focuses on delivering patient-centered care management, chronic disease support, and seamless care transitions across India. As a Care Manager, you sit at the intersection of clinical care and digital operations - supporting members throughout their healthcare journey by planning, coordinating, and monitoring care. Working closely with doctors, internal teams, and partner hospitals, the role ensures smooth care delivery, regular follow-ups, improved patient outcomes, and a consistently high-quality member experience, while also helping reduce avoidable readmissions.
