Join a purpose-driven organisation delivering high-quality home care and community services across Southern Tasmania. We’re seeking a compassionate Care Partner, Home Care to support older people living in the local community, based from our Lindisfarne office .

In this role, you’ll provide person-centred, relationship-based case management , supporting clients with low to moderate needs to live safely, confidently, and independently at home. You’ll work closely with clients, families, and local services, coordinating supports, monitoring wellbeing, and responding to changing needs through regular face-to-face and phone contact .

This is a hands‑on, community-connected role for someone who values building trust, understands the local area, and wants to make a meaningful difference close to home.
