Job Description
The Nurse Case Manager is the clinical backbone of a specialty patient program—combining patient education, benefit navigation (as applicable), scheduling/coordination, and close communication with prescribers, pharmacies, and other stakeholders. You’ll help patients start therapy successfully, maintain continuity of care, and feel supported throughout their treatment journey.
This position is a 3–6 month temporary contract with the potential to transition into a permanent role based on performance, business needs, and program requirements.
What You’ll Do (Key Responsibilities)
- Enroll & welcome patients: Complete onboarding and welcome calls following defined scripts; ensure accurate capture of clinical/admin details and consent.
- Coordinate care: Schedule diagnostics/appointments; follow up on expired or missing test results; verify next appointments; track missing post‑visit reports (where applicable).
