Case Manager

We are seeking a highly organised and hardworking Case Manager to join our team. The Case Manager will be responsible for managing and coordinating the care of children and young people, ensuring that they receive the appropriate services and resources to meet their needs.

Responsibilities

• Undertake assessments, provide case management, crisis intervention, outreach, advocacy and supported referrals to young people.
• Provide assessment, support and case management to Youth Workers
• Create and update fortnightly rosters and communicate with Youth Workers
• Review and manage incident reports
• Maximise flexibility in treatment and support services to that, where possible and appropriate, young people can maintain their current environment with minimal disruption to themselves and others.
• Actively participate in developing and implementing innovative models of service delivery in response to young peoples needs.
