22.00 - 28.00 USD Per Hour
Working Location:
Bakersfield, CA
Working Hours/Shift:
Monday - Friday 8 am to 4:30 pm

What you bring to SBHG:
High school diploma or equivalent required.Associate Degree requiredBachelor’s degree in a social science or related field preferred.(2) years’ experience in a mental health setting PreferredBilingual in Spanish preferredExperience with case management services preferred.
How you will make a difference:
Participates in initial assessments and obtains outpatient treatment provider information, options, and resources.Verifies follow-up appointments and collaborates with follow up treatment and step-down services.Conducts groups to assist patients with transition to outpatient/community-based services.Collaborates with PHF staff and caregivers to plan for and schedule discharge.Provide community resources and psychoeducation. Submits discharge packets to referred agencies.Documents all services provided in a clear and concise manner within the Electronic Medical Record within the timelines and framework established.Transport clients to and from appointments, hospital visits, etc. as needed.Develops and maintains relationships with external partners, including DBH and DSS and outpatient providers.Division/Program Overview:
24/7 inpatient involuntary program16-bed facility for acute youth (ages 5-17)Intensive treatment programs with individualized care plansAvailable for contracting Central Valley countiesMoves clients through five phases: admission; engagement; learning; stabilization and linkage to resources and aftercare
