We are seeking a dedicated and compassionate individual to join our team at a homeless shelter. The successful candidate will play a crucial role in supporting individuals and families experiencing homelessness, helping them to access the resources and services they need to achieve stability and independence.
Responsibilities
- Conduct comprehensive assessments to understand the needs and goals of clients.
- Develop and implement individualized service plans in collaboration with clients.
- Coordinate with local agencies and service providers to connect clients with necessary resources, such as housing, healthcare, and employment opportunities.
- Monitor and document client progress, adjusting service plans as needed.
- Advocate for clients within the community and with service providers to ensure they receive appropriate support.
- Provide crisis intervention and support as needed.
