Description
Comuniilfe Inc. is looking for an experienced Case Manager for its transitional residential Program in the Bronx, Safe Haven I. The Case Manager’s main responsibility is to provide comprehensive case management services to assigned clients and their family members. The Case Manager is responsible for full coordination of services for all clients assigned; ensure that all assessments and service plans are completed and all required/needed services are rendered or arranged based on service plans' specifications. Responsible to conduct room visits and inspections.
