Job Summary

As a Case Manager you will provide direct services to individuals living with mental and physical health challenges, substance use issues, poverty, and homelessness. You will work onsite and with a mobile van to support clients toward recovery and meet their housing, health, and social needs. A valid Ontario driver’s license, reliable vehicle, and insurance are required.
