    Description

    Center for Human Development (CHD), is seeking a Case Manager for our Holyoke outpatient clinic. The Case Manager provides outreach and support services to clients with psychiatric, or substance use disorder diagnoses to enable them to utilize clinical treatment services and other supports. The Case Manager assists clients to attain his/her clinical treatment plan goals in outpatient and other levels of care and works to mitigate barriers to success.
