Position Overview: The Case Manager at Rising Tide Community Initiatives plays a pivotal role in supporting individuals facing homelessness as they transition into stable housing. The Case Manager is responsible for working directly with clients to develop and implement personalized case plans that address their unique needs and challenges, including mental health, polysubstance use, and other barriers to stability.
