What You'll Do
T he Case Manager is responsible for developing and monitoring comprehensive participant service plans in collaboration with assigned participants. This includes, but is not necessarily limited to, internal and external referrals for services to stabilize medical and mental health conditions, barrier removal, job readiness coaching, job placement assistance, and retention services.


Your responsibilities will include:
+ Initiate services for participants following their clinical assessment or clinical reassessment, providing orientation to services, explaining their rights and responsibilities, and answering questions.
+ Develop and monitor wellness plans on behalf of participants with untreated or unstable medical and/or mental health conditions adversely impacting employability. This includes coordinating referrals to community-based medical and mental health providers.
