Position Summary

The Case Manager – Pathway of Hope (POH) is responsible for providing targeted services to families with a desire to break the cycle of crisis and enable a path out of intergenerational poverty. The Case Manager recruits and provides a full range of case management services.

$22.28/hour, 40 hours/week

Essential Functions (approximate time spent)

  • Provide long-term strength-based, client-centered, holistic case management to POH families (75%).
  • Maintain up-to-date, accurate client information in casefiles and through database entry (10%).
  • Recruit qualifying families for POH and promote social services and corps programs with clients and throughout the community (5%).
