Are you passionate about supporting young people as they transition to adulthood? Consider becoming a Case Manager with Thresholds' REACH program, a transitional living program that provides wraparound support to youth and young adults with mental health challenges. The Case Manager coordinates treatment, develops and implements individualized service plans, facilitates services – like school, medical providers, family, etc. – and also assists in skill building through coaching and one-on-one work with young adults ages 17-24.
ESSENTIAL DUTIES & RESPONSIBILITIES
+ Works with the transition team, including the member, their therapist, residential staff, guardian(s) and others to develop and implement a Thresholds Care Plan. Serves as the primary contact with the members’ support network to assist with the members’ goals for transitioning to adulthood.
