Caseworker – Permanency Support Program (Out of Home Care)

About the Role

Full‑time permanent position (35 hours / week) based in Dubbone with some travel across the area. As a Caseworker you’ll be a key figure in a child’s life, supporting their safety and their story. You’ll coordinate care for children and young people in out‑of‑home care with a strong focus on permanency, cultural connection, and will have control over your own schedule.

The role includes conducting regular home visits, frequent informal visits, appointments and family time. You will liaise with carers and birth families and ensure lifelong connections are made and sustained. You will develop and review case plans and risk plans, build strong culturally respectful relationships with birth families and extended guardianship outcomes, and work with the clinical team on behavior support, placement breakdowns, and sourcing safe alternative on your community connections and cultural knowledge.
