An opportunity has arisen within the ICB forcolleagues who are qualified RGN/RMN/RNLD that are currently fulfilling anon-clinical / clinical role to consider a change in direction and join the newContinuing Healthcare team.

We are looking to recruit a dynamic andinnovate Band 6 Case Manager and Assessor to work as part of the team,delivering effective case management, assessment and reviews across thegeography for persons who may be eligible for NHS funding. The successfulcandidate will be joining a newly formed integrated team of clinicians, socialworkers and administrative staff.
