Job Description

The post holder will work as part of a multi-disciplinaryteam in a patient-facing role. The post holder will take responsibility for andundertake clinical medication reviews and support patients to proactivelymanage long term conditions.

Main duties of the job

The post holder will provide primary support to generalpractice staff with regards to prescription and medication queries.

They willhelp support the repeat prescription system, deal with acute prescriptionrequests, and medicines reconciliation on transfer of care and systems forsafer prescribing, providing expertise in clinical medicines advice whileaddressing both public and social care needs of patient in the GP practice.

If the post holder is not already a nonmedical prescriber,you will be supported to develop their role to become a nonmedical prescriberand develop an area of chronic disease management within the practice.
