Community Based Services Coordinator

Position Objective:

The Community Based Services Coordinator works with and to support clients over the age of 65 years to remain in their home and be as independent as possible for as long as possible. The role is client-centred and based on the Active Service Model. The Home Services Coordinator will work alongside the NDIS Coordinator (supports clients under 65 years) as part of a multi-disciplinary Community Services team to ensure participants are supported with a suite of flexible quality services targeted to meet individual needs and expectations.

Selection Criteria:

Essential:

1. Diploma qualification in health, nursing, community services or aged care (preferred)
2. Understanding of the Aged Care Sector with particular skills and knowledge of Commonwealth Home Support Program and Support at Home Packages.
