Community Reintegration Worker - HART hub

SLCS is currently seeking a full-time Community Reintegration Worker for the HART Hub. The HART Hub (Homeless and Addiction Recovery Treatment Hub) is a low barrier treatment-oriented space offering supports to individuals who are experiencing or at risk of homelessness and who face challenges related to addiction and/or mental health to recover and improve their overall well being and secure long-term housing. As part of this collaborative and client-centered team, you will work alongside external agencies/community partners, healthcare providers, addiction specialists and participants working towards reducing barriers, connecting to resources and supporting access to safe, appropriate and permanent housing.In alignment with the HART Hub’s commitment to trauma-informed, culturally safe, and integrated care, this role will work closely with the HART Hub’s lead agency in providing wrap around supports to individuals along their journey, including offering assistance in navigating the...
