Community Support, Worker Rotary Transition Centre

The Emergency Department at St. Michael's Hospital is currently looking to hire a Community Resource Staff who will work in the Rotary Centre. Key components of the position are to provide a positive space for our vulnerable patients as they transition from the emergency department back into the community. There is a strong emphasis on strengthening the link between community agencies and the Emergency Department.

Responsibilities

• Providing respectful meaningful care for vulnerable populations using the rotary centre for respite from circumstances.
• Ensuring that the unique needs and living circumstances of the client are factored into the care, communication and discharge plan.
• Reviewing care plans and discharge plans to assess the client's capability for implementation in light of available resources and supports.
