Your Mission:

In this role, you will be responsible for anticipating and planning the user's care within the healthcare network according to their needs. You will collaborate with other professionals while taking into account the user's needs and available resources.

• You will organize both short- and long-term stays in nursing homes and ensure a smooth return home with necessary assistance and care.
