We're expanding!

Access Aged Care is Australia’s leading provider of GP and telehealth supported Aged Care. Our mission is simple: to improve the health and well-being of older Australians by combining compassionate care with innovative healthcare solutions.

We’re expanding and are looking for a dedicated Enrolled Nurse to join our Access Aged Care team from Wanneroo to the CBD.

What You’ll Do

  • Work alongside RACF care teams as the on site Access Aged Care nurse.
  • Facilitate GP rounds (both in-person and telehealth) to ensure smooth, timely care.
  • Prepare, organise, and triage referrals to maximise efficiency and outcomes.
  • Conduct bedside assessments and clinical reviews.
  • Act as the communication bridge between RACFs, GPs, and families.
  • Deliver excellent customer service, with empathy and professionalism at every interaction
  • Build strong relationships with residents, families and facility partners.
  • Use your clinical knowledge and IT skills to ensure quality outcomes.

What You’ll Bring

  • Minimum of 2 years as an EN (post graduation) within Australia, and AHPRA registration.
  • Confident communicator with critical thinking skills
  • Comfortable working autonomously
  • Strong organisational skills and attention to detail
  • Ability to build trust quickly with residents, families and teams
  • Keen to learn, and confident in your clinical judgement
  • Technologically proficient - Computer skills are a must!
  • Reliable vehicle and happy for travel to Bentley/Claremont
