Job Summary

Care & Support Executive for an Active Ageing Centre (AAC) to assist in the day-to-day operations of the centre, manage inquiries, coordinate befriender volunteer engagement, and maintain records. The role involves outreach to seniors, understand their care needs and ensuring services are referred timely.

Principal Responsibilities and Duties

Outreach & Engagement

• Plan and lead outreach strategies to engage seniors in the community, with emphasis on identifying and connecting with socially isolated seniors.
• Develop and implement targeted approaches (e.g., home visits, neighbourhood outreach, collaborations with grassroots partners) to raise awareness of centre programmes and services.
• Track outreach activities and outcomes to ensure effectiveness and continual improvement.
