Community Healthcare Network, is a Federally Qualified Health Center (FQHC) serving the underserved communities of Brooklyn, Bronx, Manhattan and Queens. Through the care we provide at our Health Centers and through our various programs, we touch the lives of 85,000 people annually. 

We are seeking a Per-Diem Family Nurse Practitioner for our South Bronx Health Center located at 1002 Westchester Ave, Bronx, NY 10459.

Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.

The ideal candidate will have a valid Family Nurse Practitioner N.Y.S. License to deliver medical care and at least two (2) year of post-graduate/residency experiencedelivering care in a Primary Care/Ambulatory Care setting.
