Family Practice - Without OB Physician

Locum Tenens Primary Care Opportunity Phoenix Arizona
Specialty: Board Certified FP or IM (will not take Board Eligible)
Client: Indian Health Service
Location: Phoenix, AZ
Dates coverage is needed: as soon as onboarding is complete 9/30 (with 6 month option to extend)
Schedule/Shifts Times: 4 ten hour shifts during the M-F week the Chief will let me know which day off during the week once onboarding nears completion, will depend on their current staffing situation. No weekends required
Call Requirements: None
% inpatient / % outpatient: Outpatient only
Patient Volume: 12- 15 patients per day average
Patient Ages: adults only
Types of Cases: standard outpatient care, chronic illness treatment
Support Staff: yes (several Physicians, mid-levels, RN s and specialty Physician support)
Procedures Required: None
Type of Charting: EMR - RPMS
Requirements: BLS (through American Heart), DEA (fee paid, schedules 2-5)
Reason for Need: staff shortage
Credentialing Timeframe: 4-6 weeks on average
Additional Info: any State License ; minimal gyn, only paps and pelvic exams for symptoms as needed. No suturing.
COVID vaccination not required
