Our team is distributed across three continents and works remotely to develop solutions for Italy, with plans to expand throughout Europe and globally. You will collaborate closely with product, operations, clinical stakeholders, and partner technical teams. Work remotely or from our offices in Milan or Sicily.


About the role


We are seeking a motivated Full-Stack Developer (mid to senior levels) with a strong focus on integrations to build and maintain robust, secure connections between Medicilio’s platform and healthcare structures, especially within the Italian public healthcare ecosystem. You’ll help design, implement, and scale integrations with hospital systems, regional health services, EHRs/EMRs, and national services, using standards like FHIR and HL7, while ensuring data security, privacy, and regulatory compliance.

Responsibilities


  • Technically Lead end-to-end design and development of integrations with healthcare providers, public health infrastructures, and third-party platforms (e.g., EHR/EMR vendors, LIS, RIS/PACS).
  • Implement and maintain standards-compliant interfaces: FHIR (R4) resources, HL7 v2 messaging, and RESTful APIs.
  • Collaborate on technical discovery with partner IT teams to assess integration feasibility, mapping, and data flows.
  • Build secure, resilient data pipelines for clinical and operational data exchange, including scheduling, orders, results, imaging metadata, and patient demographics.
  • Ensure compliance with applicable regulations and best practices (e.g., GDPR, Italian health data norms, ISO 900127001 practices, auditability, data minimization).
  • Define and enforce authentication and authorization patterns (OAuth2/OIDC, service accounts, mTLS, JWT).
  • Contribute to and lead technical planning meetings for new features and integration roadmaps.
  • Contribute to core features and microservices in our systems constantly evolving remote patient monitoring
  • Propose and implement enhancements to our codebase, integration frameworks, and developer workflows.
  • Own your domain and the interplay between frontend, backend, and integration layers; ensure robust error handling, retries, monitoring, and observability.
  • Maintain code quality and participate in peer reviews; produce clear technical documentation and data mapping specs.

 

SkillsMust Haves


We welcome candidates ranging from mid-level to lead developers. The most crucial skills we’re looking for include:

  • Demonstrated experience building and operating integrations in healthcare environments or any other compliance-heavy and complex environments
  • Strong understanding of security for healthcare data: encryption in transit/at rest, secure API design, token-based auth, least-privilege access, logging/auditing.
  • Ability to quickly learn new standards, vendor specs, and regional public health requirements.
  • Excellent communication skills with both technical and non-technical stakeholders; comfortable interfacing with partner IT teams and public institutions.
  • Positive attitude toward feedback, collaboration, and cross-functional work.
  • B+ level proficiency in Italian.


Should Haves


  • 3+ years of experience in an engineering role
  • Hands-on experience with healthcare integrations: FHIR (R4), HL7 v2.x (e.g., NextGen Connect/Mirth), custom adapters, and message brokers (Pub/Sub, Kafka, RabbitMQ)
  • Proven ability to design and operate ETL pipelines (Extract, Transform, Load) for reliable data ingestion, validation, transformation, and delivery across systems
  • Web development experience with languages such as Python (specifically Django or/and FastAPI) and Javascript (Vue or React)
  • Familiarity with PostgreSQL and its extensions (e.g. postgis) or similar database technologies
  • Systems engineering strengths in architecture, requirements definition, product fit assessment, and technology planning.
  • Practical knowledge of observability for integrations: structured logging, metrics, tracing, alerting, DLQs/retries.
Nice to Haves


  • Experience with Italian public healthcare systems and integrations with regional/national services (e.g., interoperability layers, CUP/prenotazione systems, TS/Sistema Tessera Sanitaria, Fascicolo Sanitario Elettronico/FSE).
  • Knowledge of DICOM and HL7/FHIR imaging workflows (RIS/PACS), and exposure to IHE profiles.
  • Experience with Google Cloud (GCP) security and networking (VPCs, private service connect, workload identity), or any major cloud provider.
  • Familiarity with Google OR-Tools for optimization scenarios.
  • Background in CI/CD and DevOps (Docker, Kubernetes, Terraform) and automated compliance checks.
  • Understanding of data protection impact assessments (DPIA), GDPR principles, and vendor risk assessments.
