Responsibilities
Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, oncology, cardiology. Serve as a subject matter expert in pharmacy discipline and provide guidance across pharmacies and to clinical leadership.
- The embedded Clinical Specialty Care Pharmacist is responsible for providing specialty
pharmacy care services for the clinic site(s). Serve as the subject matter expert in
pharmacy discipline and function as a key member of the multidisciplinary care team to
ensure all clinical, operational, and prescription throughput process needs are met. Assures
the appropriate selection, dosing, and administering of medication orders in specialized
areas, such as gastroenterology, dermatology, hematology, oncology, cardiology, infectious
disease, neurology, pulmonology, endocrinology, etc.
● Effectively integrate into clinic site setting and collaborate with physicians, nurses,
pharmacists, and other health care providers as a member of the multidisciplinary
team
● Communicate with physicians and other clinicians as necessary to facilitate
exemplary pharmacy care services to patients
● Demonstrate the value of our specialty pharmacy partnership to clinic site practices
● Complete patient education and comprehensive patient assessments, including
medication history, laboratory values, past medical history, pertinent physical
assessment, and patient interview.
● Performs medication therapy and disease state management, which may include
critically evaluating disease states and adjusting medication therapy to optimize
patient medication regimens and improve clinical outcomes
● Create patient profiles and maintain consistent specialty pharmacy prescription
throughput processes
● Ensure prescriptions flow through to Ascension Rx specialty pharmacy and are
identified by pharmacy staff when appropriate
● Collaboratively address financial issues to ensure patient receipt of needed
medications
● Effectively navigate patient prescriptions, including those that are either limited
distribution drugs or out-of-network
● Collect and maintain accurate data for analysis and financial reporting
● Applies the principles of continuous quality improvement consistent with job
expectations
● Review and assess medication orders to identify, prevent and resolve potential and
actual drug related problems.
● Recommend formulary alternatives when appropriate.
● Provide drug information and education to medical staff and patients.
Requirements
Licensure / Certification / Registration:
- Pharmacist obtained prior to hire date or job transfer date required. Must be willing to initiate the process to obtain one or more Pharmacist licenses within 90 days of hire/transfer for the following states: AL, AR, IA, ID, KS, KY, LA, MD, MI, MS, NE, OK, OR, TN, TX, VA & WV.
Education:
- Bachelor's or advanced degree in Pharmacy required.
Additional Preferences
Candidates who can handle a high volume of clinical pharmacist work and who have experience in either specialty pharmacy or ambulatory care (clinic-based) settings. Residency training with PGY2 being ideal.
