Monday - Friday, Days, Every other weekend<br /><br /><div style="font-size: 13px"><table cellspacing="0" cellpadding="0" align="left"><tbody><tr><td valign="top" align="left" style="padding: 0 9pt"><p class="MsoNormal" style="margin-right: 2.35pt"><b>Responsibilities</b></p><p class="MsoListParagraphCxSpFirst" style="text-indent: -0.25in"> Provides and/or assist the resident in all activities of daily living as per individual care plan.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Maintains residents stability by checking vital signs and weight and recording information</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Provides resident comfort by utilizing resources and materials.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Assist with transporting residents to different areas of facility as necessary; activities off unit, hairdresser, etc.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Answering residents call lights and request</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Encourage the resident to function at his/her maximum level by assisting in restorative techniques and encouraging as much independence as possible. </p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Attend all mandatory in-services per code, mandatory C.N.A. meetings a year and completes required hours of educational in-services to maintain certification.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Maintain appropriate records adhering to established policies, procedures, and regulations of nursing home.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Pass meal trays, assist feeding residents, and monitor meals as per policy. Pass water pitcher and nourishment. Record meal consumption, record I&O.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Interacts supportively with residents, family and other staff.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Receives and gives report to team leader at the beginning and end of shift respectively.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Responsible for carrying out facility safety and infection control program (hand washing)</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Observe and report resident needs, abnormal conditions or behavioral changes to team leader, charge nurse or nurse manager.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Completes assignment in a timely fashion. Follows resident care plan as written.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Maintains the resident s room and supplies in a neat manner.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Assist in promoting the Quality of life set forth in the Residents Bill of Rights .</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Carries resident assignment sheet on person while on duty. Does not leave sheet where available for others to read. States or able to locate information i.e. resident C.P.R. status. </p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Respects the confidentiality of patients, departments, and nursing home information.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Reports to work on time and is on the unit and ready for duty at 7:00 am, 3:00 pm, or 11:00 pm. </p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Gives adequate notice to supervisory personnel when calling in sick or late, according to department standards.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Lifts and transfers residents as per individual care plans.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Punches in and out, consistent with nursing home policy.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Observes department guidelines regarding use of meal periods, coffee, breaks, and quitting time. Signs out and in at meal time.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Does not abuse sick time when used, is verified as required.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Resolves conflicts or problems in working relationships without involvement of higher authority.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Safety rules and procedures are followed. Works carefully and avoids situations that could lead to and accident.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> All accidents are reported to supervisor upon occurrence, an incident form is completed and sent to DNS.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Work area and equipment are kept clean and orderly.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Utilizes personal protection equipment, engineering and work practice controls to prevent/minimize occupational exposure to blood and other potentially infectious materials.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Works with supervisor and other members of the work group in planning and building an effective team.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Instructions are followed, work schedules and standards met without close supervision.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Work is sufficient quality and quantity.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Deviations from established schedules and instructions are approved by nurse manager or their designee.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Interacts with family members and other staff in a professional manner. Promote positive environments.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Adheres to dress code at all times. </p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Demonstrates a willingness to learn new procedures by asking questions and/or demonstration.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Adheres to shift routine.</p><p class="MsoListParagraphCxSpMiddle" style="text-indent: -0.25in"> Clean closets.
