This position is a supervisory position assisting in oversight and delivery of non-medical home care services to our clientele who desire to age in place at home.

  • Conduct in-home Client Assessments and develop individualized Care Plans
  • Supervise ongoing care of Client
  • Conduct Quality Assurance and ongoing Reassessment of clientele under the MDH Home Care Statutes and Regulations
