Job Description

We are seeking a dedicated and compassionate Home Training Nurse to join our healthcare team in Louisville. This role is pivotal in providing home-based training and support to patients requiring ongoing medical care or rehabilitation services. The ideal candidate will have a strong background in nursing, excellent communication skills, and a passion for patient education and empowerment.

Key Responsibilities:

  • Conduct individualized training sessions with patients and their caregivers to teach safe and effective home hemodialysis techniques.

  • Assess patient readiness for home hemodialysis and develop personalized training plans to address specific needs and learning styles.

  • Monitor patient progress and provide ongoing support, education, and encouragement to ensure successful adaptation to home treatment.

  • Collaborate with the healthcare team to coordinate care and ensure continuity of services for patients transitioning to home hemodialysis.

  • Maintain accurate and detailed records of patient training sessions, progress, and any changes in treatment protocols.

  • Participate in quality improvement initiatives to enhance home hemodialysis program effectiveness and patient outcomes.

    • Qualifications:

  • Registered Nurse (RN) with a current state license to practice in Kentucky.

  • PD Highly preferred

  • Minimum of 2 years of experience in dialysis nursing, with prior experience in home dialysis training preferred.

  • Strong clinical assessment and patient education skills.

  • Excellent communication and interpersonal skills.

  • Ability to work independently and as part of a collaborative team.

  • Proficient in using electronic medical records and technology relevant to home dialysis.
