Responsibilities:
- Provide compassionate and quality care to hospice patients and their families
- Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Administer medications and treatments as prescribed by the physician
- Monitor and evaluate patient's response to treatment and adjust care plan as needed
- Educate patients and families on disease process, symptom management, and end-of-life care options
- Provide emotional support and counseling to patients and families
- Coordinate with other healthcare professionals to ensure continuity of care
- Document patient care activities accurately and timely


Qualifications:
- Valid RN license in the state of Illinois
- Minimum of 1 year of nursing experience, preferably in hospice or palliative care
- Knowledge of infection control principles and practices
- Familiarity with sterile processing techniques
- Experience in family planning and counseling
- Ability to lift heavy objects and assist with ambulatory patients
- Familiarity with dementia care techniques
- Knowledge of medical imaging procedures and interpretation
- Experience with esthetic laser treatment is a plus
- Familiarity with durable medical equipment (DME) usage and maintenance
- Proficiency in sonography is a plus
