About the Job:

• To provide human services to clients with various needs and challenges.


• To work collaboratively with nurses, healthcare professionals, and other stakeholders in a socio-health context.


• To conduct assessments of client needs, develop intervention plans, and implement them while respecting legal boundaries.


• To offer support, guidance, and resources to clients facing crises or urgent situations.


• To evaluate the effectiveness of care pathways for each intervention and make recommendations for improvement.

The ideal candidate will have strong communication skills,
broad knowledge of social health networks,
