We are currently hiring a part time Intake Coordinator LVN for NOC shift at Fremont Hospital!

NOC Shift: :PM - 7:AM

As an active member of the Assessment & Referral Team, the Intake LVN performs the Initial Psychiatric Screening Assessments for incoming patients and coordinates with the Medical Staff in directing the patient towards an appropriate level of care.

    Qualifications

    Education:

  • AA/AS Degree or Bachelor’s Level Degree preferred.
  • Master's in social work, psychology or related mental health field, PhD and PsyD candidates in terminal degree programs with Masters equivalent or a degree from an accredited program in nursing preferred.

    • Experience:

  • A minimum of one (1) year direct clinical experience in a psychiatric or mental health setting, with direct experience in patient assessment, family motivation, treatment planning, and communication with external review organizations.

    • Licensure:

  • LVN licensure required.

    • Additional Requirements:

  • CPR with AED (CPR/AED) certification must be current and presented to Human Resources upon hire.
  • You must successfully complete the Fremont Hospital’s non- violent Crisis Prevention Intervention (CPI) training course within days of employment and prior to assisting with preventative intervention measures.
  • You may be required to work overtime and flexible hours as needed.
