Summary

Join CentraCare’s Integrated Behavioral Health team and provide outpatient assessment and brief intervention services within the Coordinated Care Clinic, a program designed to support patients with complex medical, behavioral health, and social needs who experience frequent hospitalization. This role focuses on integrated primary and mental health care, medication and substance use support, social and community resource coordination, and continuity of care for individuals transitioning from incarceration back into the community.
