  • Primary care and chronic disease management comprises the bulk of care.
  • Responsible for assessment, diagnosis, and management of healthcare needs, utilizing standardized and evidenced-based procedures.
  • EPIC EMR
  • Ambulatory (outpatient) and mobile clinic practice. 

    • Details:

  • Full-time: 4 or 5 days/week
  • Monday – Friday, 8AM – 5PM
  • Urgent Care – internal medicine / urgent care physician, as the site will focus on helping patients transitioning from inpatient hospital care to care to outpatient setting elsewhere 

    • Benefits:

  • 240k – 265k / year DOE
