Learning Disability Liaison Nurse

We are seeking a compassionate and experienced Band 6 Learning Disability Liaison Nurse (LDLT) to join our dynamic multidisciplinary team. The post holder will work closely with acute hospital teams across Sussex to ensure that people with a learning disability receive equitable, safe, and effective care during inpatient admissions. This role plays a critical part in implementing the learning from LeDeR reviews and meeting the trust's responsibilities to reduce health inequalities and improve health outcomes for people with a learning disability. The LDLT nurse will act as a specialist advisor, supporting hospital staff to make reasonable adjustments, improving understanding of complex communication needs, and ensuring person-centred care planning.
