This Licensed Practical Nurse (LPN) position provides Case Management to clients living in the Airdrie area. This position requires advanced critical thinking abilities and a wide-ranging interventions skill set & experience. In collaboration with the with the interdisciplinary team, the Licensed Practical Nurse is responsible to assess, plan, implement and evaluate the needs of Home Care clients and their caregivers/families. The Licensed Practical Nurse is responsible for their nursing practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA). There is also an expectation to be a mobile worker.
