River Falls Senior Living is seeking a compassionate Licensed Practical Nurse (LPN) to join our Assisted Living community. This role focuses on resident wellness, care coordination, and collaboration with care staff in a supportive, team-oriented environment.
As an LPN, you will work with the interdisciplinary team, resident, and families to assess needs, develop, and implement Individual Service Plans (ISPs). You will partner with the Wellness Director and Resident Care Assistants to ensure optimal care and quality of life for each resident.
